Please note that a request for placement of a child at the Glenleighden School does not automatically ensure a place.
The criteria for a placement at the Glenleighden School is very specific and requires a diagnosis of a severe communication disorder and / or an ascertainment or a profile (Education Adjustment Process) of Speech Language Impairment at levels 5 or 6, approved by an external educational authority. (i.e. Education Queensland, ISQ or Catholic Education) for children over 6 years of age. It is important to note that places allocated are dependent on vacancies, availability of government funding and other factors.
Essential Information Required
A Referral Form 2009 V3.pdf must be completed. Referrals can be made by parents, educators and other professionals.
If your child has previously seen a Speech Therapist or Speech Language Pathologist please attach a copy of any reports/assessments completed and a history of intervention.
Please attach any other relevant reports/assessments that have been completed on your child. These may include:
- Cognitive Assessment
- Occupational Therapy
- Physiotherapy
- Psychology
- Paediatric report
- Hearing test results
- Most recent Educational report
- Current IEP
It is important that this essential information is available to the Committee so as a profile of the child’s current needs can be generated. The lack of any of this detailed information can mean that further requests and follow up to you by the committee may delay the process and could impact on the timeline in which an appointment may be made for your child.
Please also note that whilst attendance at a consultation would provide us with information about your child’s needs, it is neither part of our Full Diagnostic Assessment nor school entry processes.
